Letters of Recommendation:
You will need two letters recommending you for sex reassignment surgery (vaginoplasty, phalloplasty), one must be by someone with a Ph.D. and the other should at least have a master's degree. These letters can be from a therapist, counselor, psychiatrist, psychologist or social worker. If you transitioned over five years ago, you may only be required to have one letter.
Dr. Bowers only requires one letter for an orchiectomy. There are no letter requirements for a hysterectomy.

You must have been on hormones for at least a year before surgery.
Real Life Experience:
Dr. Bowers requires for you to have lived full time for over a year in the new gender role before SRS.
You should notify Dr. Bowers of any major medical problems or prior surgeries. If you take medications, they will need to be disclosed as well.
Heart problems are of particular concern. For liability reasons you will need to provide a letter of surgical clearance from a cardiologist.
You may also be required to have labwork or testing done prior to surgery. Diabetics must submit labwork within 30 days of surgery. Please contact Dr. Bowers if you have any of these health conditions for additional information and requirements.
You must weigh less than 210 pounds for SRS with Dr. Bowers. There might be an exception if you are tall or have a lot of muscle.
Dr. Bowers requires for you to be a non-smoker.
You will need to have a HIV test submitted to Dr. Bowers no earlier than 12 weeks before MTF SRS. You can still most likely get surgery if HIV positive if you are in good health. This must be discussed with Dr. Bowers though. FTMs aren't required to have testing.
Dr. Bowers doesn't require hair removal before MTF sex reassignment surgery. She highly recommends it though due to the fact that hair grows in cycles and so they can't be removed all at once during surgery. She suggests using laser if you have dark hairs because it can be much faster and cheaper.
You should focus on removing hair on the scrotal sac and penile shaft area. The middle 2/3 of the scrotum area is the most important. You can leave some hair along the sides for possible scar cover-up.
If you want to avoid professional services, but don't want to risk hair growth in your vagina you have a few options. You can try the Tria laser hair removal system (www.triabeauty.com) or Silk'n (www.silkn.com). Tria has worked for me, but only on black hairs.
You also may be able to do some of your own electrolysis from home with the One Touch system. The cheapest place to buy it is on Ebay, it usually costs about $10 USD. Make sure to also get replacement tips though, they bend easily. Please start out on a low setting to see how you heal and avoid scarring.
I have found Tend Skin to work very well to reduce redness and bumps after electrolysis.
